ABOUT OPTIONAL MBNA
®
CREDIT CARD PAYMENT PROTECTION PLAN

ENROLLMENT:
You are eligible for enrollment in optional MBNA Credit Card Payment Protection Plan group creditor insurance if you are a Primary Cardholder who is at least 21 years of age and less than 66 and a Canadian resident.
Your Certificate of Insurance, which indicates your effective date of coverage, will explain all benefits and terms of coverage including exclusions and limitations, and will be sent to you upon enrollment. If you choose to enroll, you have the option to cancel MBNA Credit Card Payment Protection Plan at any time. If you cancel this coverage within 30 days of receipt of the Certificate of Insurance any premiums you have paid will be refunded. If you have any questions regarding this insurance or wish to cancel this coverage, please call Assurant at 1-800-340-4717.

COST:
$1.20 per $100
of your Insured Balance, plus applicable taxes, to a maximum of $25,000. The cost for coverage will be charged to your credit card account. Insured Balance is defined as outstanding account balance, calculated as of the Statement Date if you reside in a Province/Territory other than Quebec, or as Average Daily Balance if you reside in the Province of Quebec as of the Statement Date. Average Daily Balance is calculated by adding the credit card account balance each day and dividing that total by the number of days in the statement period. Statement Date means the last date of the statement period for which Your Account statement is produced.

Example:
For example, in a Province/Territory other than Quebec, an account with a $500 outstanding account balance as of the statement date would incur a cost for coverage of $500 x $1.20 per $100 = $6.00 plus applicable taxes.

In the Province of Quebec, if your starting balance is zero and you charge $1,000 to your credit card in the middle of your billing cycle, your average daily balance would be half, or $500. Applying the premium rate of $1.20 per $100, you would see a charge of $6.00 on your statement, plus applicable taxes.

SUMMARY OF BENEFITS, EXCLUSIONS AND LIMITATIONS

Life Coverage:
This coverage may pay a one-time benefit payment to your credit card account should you die. The lump sum payment is based on the amount owing on your credit card account as of your date of death, to a maximum of $25,000.
Benefits are not payable in some situations.
For example, we will not pay a Life benefit within 6 months of the effective date of coverage if death results from suicide.

Total Disability Coverage:
This coverage may pay a monthly benefit payment to your credit card account if you become totally disabled as a result of an accidental bodily injury or sickness, and the disability lasts for more than 30 consecutive days. Monthly benefit payments will be made and will be equal to the greater of 20% of the outstanding balance as of the statement date coinciding with, or if not applicable, immediately before the Date of Loss, and $10. Monthly benefit payments will be made for every month of total disability until the eligible balance has been paid, to a maximum of $25,000. Features include a monthly refund while receiving benefit payments of the premium charged as of the statement date coinciding with or immediately before the date of loss, and an additional one-month benefit payment may be paid when you are no longer disabled and return to performing your normal activities of daily living.

Job Loss Coverage:
This coverage may pay a monthly benefit payment to your credit card account if you experience a Job Loss and remain unemployed for more than 30 consecutive days. To be eligible for Job Loss benefits, you must have been employed in a non-seasonal occupation with the same employer for at least three consecutive months immediately prior to the Date of Loss. Monthly benefit payments will be made, and payment will be equal to the greater of 20% of the outstanding balance as of the statement date coinciding with, or if not applicable, immediately before the Date of Loss, and $10. Monthly benefit payments will be made for every month of unemployment until the eligible balance has been paid, to a maximum of $25,000. Features include a monthly refund while receiving benefit payments of the premium charged as of the statement date coinciding with or immediately before the date of loss, and an additional one-month benefit payment may be paid upon your return to work.
Benefits are not payable in some situations.
For example, we will not pay Job Loss benefits if your Job Loss is due to or results from: loss of self-employment, resignation, loss of seasonal employment, dismissal for cause, labour disputes, strikes or lockouts, disability, or any reason within 30 days of the Effective Date.

All insurance coverage automatically ends when the group master policies are cancelled, your credit card is no longer in good standing or cancelled or you pass away. Life coverage terminates on the next account statement date after you turn 80. Total Disability and Job Loss insurance coverages terminate on the next account statement date after you turn 66.

There are time limits to submit a claim. You must claim within 90 days of the insured loss, except for Life claims which should be submitted as soon as reasonably possible. The insurer pays approved claims within 30 days of receiving the proof required. If your claim is denied, you will receive a written explanation with the appeal process.
