CreditWise balance protection insurance

DISCLOSURE STATEMENT ABOUT OPTIONAL MBNA ®  CREDITWISE PLUS ®  INSURANCE 1  
ENROLLMENT:  You are eligible for enrollment in optional MBNA CreditWise Plus group creditor insurance if you are a Primary Cardholder between the ages of 18 and 69 and a Canadian resident. Your coverage begins on the date your application is approved. Your Certificate of Insurance, which indicates your effective date of coverage, will explain all benefits and terms of coverage including exclusions and limitations, and will be mailed to you upon enrollment.  In the meantime, if you would like to retain this important information about optional MBNA CreditWise Plus, please print prior to completing your application. If you choose to enroll, you have the option to cancel MBNA CreditWise Plus at any time. Coverage may be null and void if there has been a material misrepresentation of any kind. If you cancel this coverage within 30 days of the Certificate of Insurance's delivery, any premiums you have paid will be refunded. If you have any questions regarding this insurance or wish to cancel this coverage, please call Assurant at 1-800-340-4717. Note that there are other products on the market offering coverage similar to the CreditWise Plus insurance plan. You may want to verify if you are already covered by such a product.
COST: $1.20 per $100 of your average daily balance per month, plus applicable taxes, to a maximum of $20,000. The cost for coverage will be charged to your credit card account.
Example:  An account with a $1,000 average daily balance would incur a cost for coverage of $1,000 x $1.20 per $100 = $12.00 plus applicable taxes.
SUMMARY OF BENEFITS, EXCLUSIONS AND LIMITATIONS 1  
Life Coverage: This coverage may pay a one-time benefit payment to your credit card account should you or your spouse die. The lump sum payment is based on the amount owing on your credit card account as of your date of death, to a maximum of $30,000. Benefits are not payable in some situations. For example, we will not pay a Life benefit within 6 months of the effective date of coverage if death is due to intentionally self-inflicted injuries or suicide while sane or insane.
Accidental Death Coverage:  This coverage may pay a one-time benefit payment to your credit card account should you or your spouse die as a result of an accident after your 70th birthday. This coverage replaces Life coverage when you turn 70. At age 70 all other coverages terminate and your monthly premium rate reduces to $0.59 per $100 of your balance as explained in the "Cost" section above. The lump sum payment is based on the amount owing on your credit card account as of your date of death, to a maximum of $30,000.
Accidental Dismemberment Coverage:  This coverage may pay a one-time benefit payment to your credit card account should you or your spouse suffer a covered dismemberment directly due to accidental bodily injuries. The lump-sum benefit payment is based on the amount owing on your credit card account of as your date of loss, to a maximum of $30,000.
Critical Illness Coverage:  This coverage may pay a one-time benefit payment to your credit card account if you or your spouse are diagnosed with a covered Critical Illness, such as heart attack, stroke, cancer, coma, coronary artery disease (refers only to bypass surgery), major organ transplant, multiple sclerosis, muscular dystrophy, paralysis, renal failure, Alzheimer's or total blindness. The lump sum benefit payment is based on the amount owing on your credit card account as of your date of diagnosis, to a maximum of $30,000. Benefits are not payable in some situations.  For example, if you or your spouse are diagnosed with a Critical Illness which was diagnosed prior to the effective date of insurance and have not completed a period of at least 180 consecutive days free of all symptoms, medical treatment/services including taking drugs/medicine for such disease or condition.
Loss of Self-Employment Coverage:  This coverage may pay a monthly benefit payment to your credit card account if you experience a period of unemployment due to the forced closure of your business due to creditor induced bankruptcy and remain unemployed for more than 30 consecutive days. To be eligible, you must have been self-employed at least 25 hours per week for an active business that had been incorporated for a period of 24 consecutive months prior to the date of loss. Monthly benefit payments will be made and for the initial payment will be equal to the greater of 10% of the Eligible Balance 2  and the Minimum Payment 3  and for subsequent months will be equal to the greater of 10% of the Eligible Balance 2  and $10. Monthly benefit payments will be made for every month of unemployment until the Eligible Balance 2  has been paid, to a maximum of $30,000. Features include, a monthly refund while receiving benefit payments of the premium charged as of the statement date coinciding with or immediately before the date of loss, and one additional monthly benefit payment may be paid upon your return to work. Benefits are not payable in some situations.  For example, if your Loss of Self-Employment is due to: voluntary forfeiture of salary, wages or income; closure of business due to willful misconduct or criminal misconduct; or bankruptcy within the first 12 months of the effective date.
Total Disability Coverage:  This coverage may pay a monthly benefit payment to your credit card account if you or your spouse become totally disabled, and the disability lasts for more than 30 consecutive days. Monthly benefit payments will be made and for the initial payment will be equal to the greater of 10% of the Eligible Balance 2  and the Minimum Payment 3  and for subsequent months will be equal to the greater of 10% of the Eligible Balance 2  and $10. Monthly benefit payments will be made for every month of total disability until the Eligible Balance 2  has been paid, to a maximum of $30,000. Features include, a monthly refund while receiving benefit payments of the premium charged as of the statement date coinciding with or immediately before the date of loss, and one additional monthly benefit payment may be paid upon your return to work.
Involuntary Unemployment Coverage:  This coverage may pay a monthly benefit payment to your credit card account if you experience a period of Involuntary Unemployment, and remain unemployed for more than 30 consecutive days. You must have been employed at least 25 hours per week for at least 90 consecutive days prior to the date of loss for an employer other than yourself. Monthly benefit payments will be made and for the initial payment will be equal to the greater of 10% of the Eligible Balance 2  and the Minimum Payment 3  and for subsequent months will be equal to the greater of 10% of the Eligible Balance 2  and $10. Monthly benefit payments will be made for every month of unemployment until the Eligible Balance 2  has been paid, to a maximum of $30,000. Features include, a monthly refund while receiving benefit payments of the premium charged as of the statement date coinciding with or immediately before the date of loss, and up to one additional monthly benefit payment may be paid upon your return to work. Benefits are not payable in some situations.  For example, if your Involuntary Unemployment is due to: retirement, voluntary forfeiture of salary, wages or income, or termination as the result of willful or criminal misconduct.
Disability Requiring Hospitalization:  This coverage may pay a benefit to your credit card account if you or your spouse is hospitalized as the result of accidental bodily injury or sickness for at least 24 hours, based on the greater of 10% of your Eligible Balance 2  and the Minimum Payment 3 . This coverage may also provide a one-time benefit payment to your credit card account of up to $30,000 if your hospitalization continues for more than 30 consecutive days. Benefits are not payable in some situations.  For example, no benefit will be paid where the hospitalization is in a convalescent, nursing, rest or skilled nursing facility or a facility that operates exclusively to treat the mentally ill, aged or those suffering from drug addiction or alcoholism, or a hospital outside North America, nor will benefits be payable if the hospitalization results from intentionally self-inflicted injury, normal pregnancy, commission or attempt to commit a criminal offense, elective cosmetic surgery or cosmetic or beauty treatment unless directly related to a physical illness, disease or injury.

There are time limits to submit a claim. You must claim within 90 days of the insured loss, except for Life claims which should be submitted as soon as reasonably possible.

3. Minimum Payment means payment due on your account as of the statement date coinciding with, or if not applicable, immediately before the date of loss.

Underwriters:  Life, Accidental Death, Accidental Dismemberment and Critical Illness coverages are underwritten by American Bankers Life Assurance Company of Florida (ABLAC) and Loss of Self-Employment, Total Disability, Disability requiring Hospitalization, and Involuntary Unemployment coverages are underwritten by American Bankers Insurance Company of Florida (ABIC). ABIC, ABLAC, their subsidiaries, and affiliates carry on business in Canada under the name of Assurant® under policy forms WFR1 (01/16) R and LUZ (01/16) R. ®Assurant is a registered trademark of Assurant, Inc.
